New Client FormIf you are a new client who is looking to book for extensions, color or highlights, you are required to fill out this form and book an in-person consultation appointment before we are able to book your official appointment. If unable to fill out digital form, please come in 5 minutes before your consultation appointment to fill it out.

Cancellation & Rescheduling Policy
Your reservations are very important to our stylists at Midori Eco Salon and the time is reserved exclusively for you. We completely understand that sometimes schedule adjustments are necessary; therefore, we have established the following cancellation/no-show policy. Please understand that when you forget to cancel or reschedule your reservation, we miss the opportunity to accommodate guests on our waiting list.
NEW GUESTS
When making your first reservation, it will be considered tentative. You MUST confirm or cancel your reservation 24 hours in advance to remain scheduled. If you fail to confirm this reservation, it will be deleted and you will have to reschedule. A fee of $50 will be applied for all new guest reservations cancelled less than 24 hours in advance.
RESERVATION REMINDER
As a courtesy, Midori Eco Salon sends a reminder via text 48 hours prior to your reservation. Midori Eco Salon ALSO gives a courtesy call 24 hours prior to your reservation. If we are unable to reach you, or unable to leave a voicemail, please understand the ultimately you are responsible for your reservation.
Midori Eco Salon may require a credit card to HOLD the reservation.

1. made at least 48 hours prior to the appointment will not be charged any penalties.
2. made with fewer than 48 hours will be charged 50% of the total service(s).
3. no shows will be charged 100% of the service(s) fees.
*Midori may consider extenuating circumstances.
The cancellation policy allows us time to inform our guests on standby of availability and also helps to keep our stylists’ schedules filled. Midori Salon’s policies are provided to ensure quality of service and an excellent experience to our established guests and to our future guests.
